HOW TO ENTER THE DRAWING
The following explains, in complete detail, the rules of the drawing and how to have your name entered to win a cruise for two from Duncan Chiropractic Group.
For each new qualifying patient your refer to Duncan Chiropractic Group, you will be eligible to have your name entered into the drawing and receive 5 tickets to do so. There is no limit on the number of patients you may refer or the number of entries you may have in the drawing. The more new patients you refer, the more entries you will receive.
To be considered a “qualifying” referral, each new referral must schedule and keep an appointment with duncan chiropractic group for a chiropractic exam. Referrals who fail to schedule an appointment, or who fail to keep a scheduled appointment, will not be counted as a qualifying referral. Referrals are considered new patients who are not a current patient of record with duncan chiropractic group.
Referred patients must schedule an appointment and complete their appointment NO LATER THAN April 25th, 2019. The winning entry will be chosen on April 26th, 2019.
A patient referred for the purposes of this giveaway cannot be a current patient of duncan chiropractic group.
You do not have to be a current patient of duncan chiropractic group to be eligible to refer new patients and enter this giveaway.
If you are not a current patient of duncan chiropractic group, you may refer yourself as a qualifying new patient for the purpose of entry.
Additionally, you may post a review about your positive experience with our office on either Google, facebook, healthgrades and/or Yelp. For each review that is verified by our staff you will receive 3 tickets to be entered into the raffle.
You may also choose to record a testimonial video of your experience and success with treatment at duncan chiropractic group. By recording a video testimonial you give us permission to use it on our websites, social media pages and other marketing purpose. Each video testimonial earns you 3 tickets to be entered into the raffle.
Employees, vendors, contractors and other service providers for duncan chiropractic group, and their families, are ineligible to enter this giveaway.
TERMS AND CONDITIONS OF THIS GIVEAWAY
Duncan Chiropractic Group will award one winning entrant, two $600 gift cards to any cruise line of your choice. This gift will have no cash value other than that outlined in the gift card. Duncan Chiropractic Group will not be obligated for any further expenses incurred by use of this travel certificate and bears no responsibility for additional charges above the cost of the gift cards included in this giveaway
